Passport, Visa, and Health Requirements

Fill out and submit the form below to determine the travel documents and immunizations needed. Only one form needs to be submitted if the country issuing the passports is the same for all travelers. The form must be reset and resubmitted for each traveler from differing countries.

Note: U.S. citizens and foreign nationals are required to carry a passport when traveling to and from the U.S. This is not an application for a passport or visa.

Type of passport held. If you are unsure, please contact your local state department.

	Normal (most common)
	Seaman (merchant seaman only)
	Government duty (government employees only)

*Destination Points Countries you will travel to during your trip. Use ctrl-key to select or deselect multiple destinations.

Transit Points Countries through which you are connecting en route to your destination. Use ctrl-key to select or deselect your transit points.

Countries Visited in the Last 6 Days Select the countries in which you have been during the last six days prior to your arrival in your destination. Use ctrl-key to select or deselect visited countries.
